Health Advocate Preliminary Consultation

Service Description

Designed to help support your journey to wellness and help you access the health care you need. As an independently-contracted personal health advocate and nurse educator, Antonette provides client-specific phone consultations and videoconferencing services to help individuals stay educated on health resources, diagnoses, test results, and medical language pertaining to their health, as well as advocate for health resources that may have otherwise been difficult to access or navigate in order to improve and maintain their health and wellbeing. *A 45-minute preliminary consultation must be booked prior to starting Health Advocate Services to assess that the health advocate is a good fit for Client's needs. If determined that a successful Client-Advocate relationship can be established, a custom package will be designed according to Client needs. The indicated price is for the preliminary consultation ONLY. In some instances, services may be achieved within this consult. If additional Health Advocate services are determined as needed, the price of the preliminary consult may be rolled into the final package cost for total Health Advocate Services. DISCLAIMER Neither the Company nor its agent, Antonette Montalvo, are the Client’s clinician, therapist, public relations manager or business manager. There is no guarantee that Client will achieve all health goals using the techniques, ideas and materials presented in the Service. Client understands that a business relationship does not exist between the parties after the conclusion of the Service. If the parties choose to continue their relationship, a separate agreement will be entered into. CLIENT RESPONSIBILITY The Service is developed for strictly informational purposes. Client accepts and agrees that Client is 100% responsible for his or her progress and results from the Service. Consultant makes no representations, warranties or guarantees verbally or in writing as to results. Client understands that because of the nature of the service, the results experienced by each client may significantly vary. Client acknowledges that as with any business endeavor, there is an inherent risk of loss of capital and there is no guarantee that Client will reach his or her goals as a result of participation in the Service.

Cancellation Policy

There is no guarantee that Client will earn any money using the techniques, ideas and materials presented in the Course. REFUNDS We have so much confidence in our ability to help you unlock your potential that all sales are final upon payment. However, if in the event you decide the Course did not meet your expectations, the Consultant is willing to reasonably remediate the situation with the Client, within a appropriate timeframe (no less than 1 day, no more than 28 days), based upon written and agreed upon reasonable measures, determined mutually by both the Consultant and Client.
